Registrations for the 2017-2018 Parish Religious Education Program are now closed.

Please carefully and completely read and complete the registration forms as promptly as possible. All registrations received after July 30, 2017 will be placed on a waiting list and are subject to classroom and volunteer availability.

Traditional (Paper) Registration

If registering using paper forms, you can print the forms from the links below or they are available on the Information Table outside the kitchen under the PREP Family Mailboxes.

PREP Registration Fees

Family Registration Form: One form is required per family.

Student Registration Form: One form is required per student. We are required to maintain this information for the health and safety of all students. Information is kept confidential. This is information we are required to obtain, similar to school emergency cards that are required to be completed and signed annually.

Volunteer Registration Form: For anyone interested in assisting our program by serving as a classroom catechist or aide, you are eligible for up to a 50% discount in fees (at discretion of pastor and DRE). If you are returning volunteer, a signed volunteer form is required to be kept on file for each year you are active. Requirements: A willingness to share and grow your own faith as well as completing a Protecting God’s Children awareness session and an Ohio BCI background check (or FBI check if not a resident of Ohio for at least five (5) consecutive years).

Payments by check or cash may be mailed or dropped off at the Parish Religious Education Office during business hours or by appointment, or may be placed in the collection basket when your family attends Mass. If placing in the Sunday collection, be certain to place in a separate envelope clearly marked “P.R.E.P. 2017-2018”

All credit/debit card payments must be made via our online giving site at https://stelizabethchurch.weshareonline.org/P.R.E.P.Fees

Special Circumstances:

If there are any questions, request home schooling option (for non-sacramental years), you have a student with special circumstances, for example, if you have a 7th grader that has not received First Eucharist; or have any other needs, please contact the Director of Religious Education, Mr. Dave Gruber.
